Consent Form

In order for the Office for Persons with Disabilities (OPD) to provide support services and accommodations, UW staff must consult and coordinate activities collaboratively. Therefore, advisors may occasionally need to share information regarding your support needs and/or accommodation arrangements.

In addition, it may be helpful to share information regarding your learning accommodation with your instructors in order to help them better understand your academic accommodations.

To submit your form, you require Adobe Reader.  If you do not have Adobe Reader, click on the icon to download the software.

Next:

  1. Click on the Consent Form sublink above the Adobe Reader icon to download the pdf file
  2. Open the file using Adobe Acrobat Reader
  3. Print and complete the form
  4. Mail, fax (519-746-2401), or drop the form off in person to the OPD (Needles Hall, room 1132)
