To meet your needs, the Office for Persons with Disabilities (OPD) works in conjunction with the Distance Education Office to provide course materials in alternative formats (e.g. large print, text on tape, Braille, etc.) and to provide suitable exam arrangements.
Many distance education courses are offered via audio (mp3 and CD-ROM) and printed material; an increasing number are available online. If you have any needs related to a physical, medical, psychological, sensory, or learning disability that require alternative formatting of course materials and/or exam arrangements, please let us know and register with the OPD by completing this form.
You will need to complete this form each term to let the OPD know of your new course(s).

Please note that all students registering with the OPD must submit verification of their disability to receive services (e.g. medical documentation, psycho-educational assessment). All documents are considered confidential and kept on file at the OPD. A signed OPD Information form gives UW staff the right to release and exchange any information necessary to provide academic accomodations on your behalf.
